MyEvaluations On-line patient log H&P's
MyEvaluations
I want to look at a past evaluation but when I went in again, I could not access it.
Check the dates of the period you are requesting. It must be within the time frame of the rotation the evaluation was based on.
Will my evaluation of my attendings or residents be seen by them?
Yes, eventually and anonymously. The information will only be sent to the attending or resident after at least 5 evaluations have accumulated. The attending or resident will receive the information as anonymous evaluations and comments a long time after you have completed the rotation.
On-line patient log
Do I have to enter all my patients into the log?
YES!! We are required by the LCME accreditating body to verify the number and type of patients the students are seeing during their Pediatrics rotation.
Use one of the following classifications for all patients:
Adolescent
Cardiology
Critical Care (not ED)
Emergency Medicine
Endocrinology
Gastroenterology
Genetics
Hematology/Oncology
Infectious Disease
Nephrology
Neurology
Newborn Nursery (well-baby only)
Patient Cases
Pulmonology
Well-child care (usually clinics)
H & P's (History & Physical Exams)
Where do I get a cover sheet?

How many H&P's do I have to complete?
Four total: 1 from your preceptor; 1 from an inpatient attending; and 2 from senior inpatient residents (not interns!) What is the deadline?
Check your orientation packet for exact dates. Normally, your H&P's must be in the possession of the evaluator by 6:00 PM on the last Wednesday before the end of the block. This means in their hands NOT in inner-office mail.
Your H&P will be returned to you if you hand it in to your preceptor/attending/resident after that time, and you will receive a zero for that portion of your H&P grade.
Can an intern (1st-year housestaff) do one?
NO! Two must be from senior inpatient residents.
