While the BUN and creatinine confirm the previous diagnosis of significant renal insufficiency,
they do not point to a cause (may types of renal disease ultimately yield elevated BUN and
creatinine). The elevated serum sodium with a urine specific gravity of 1.007 (plasma is 1.01)
is paradoxical--the plasma is more concentrated than it should be, indicating dehydration, but the urine is quite
dilute (the urine should be more concentrated than usual in an attempt to conserve water and
dilute the plasma). These findings point to a serious and unusual problem with fluid balance.
