Axial non-contrasted CT scans at the level of the lateral ventricles and centrum semiovale show wedged-shaped low density in the left middle cerebral artery (MCA) territory representing acute edema. Note that the cerebral sulci are effaced compared to the right. The findings are pathognomonic of an acute MCA territory infarct. Diagnosis: acute left middle cerebral artery infarction.
