This is a close up of the ventral surface of the brain at the Sylvian fissure. Tip of the right temporal lobe has been amputated to show the blood vessels in the Sylvian fissure. The right carotid artery, posterior communicating and anterior cerebral artery are normal. The right middle cerebral artery at the distal bifurcation shows a ruptured sacular aneurysm, yellow arrow . Diagnosis: Right middle cerebral artery ruptured aneurysm.
