The left hemisphere shows a localized hematoma that involves the putamen and part of the anterior limb of the internal capsule. The hematoma has not ruptured into the ventricle and has spared the insular cortex. Diagnosis: Spontaneous hypertensive hemorrhage of the left putamen.
