S5C29P22-Mucinosis

S5C29P22-1: This lymphocytic infiltrate of the dermis shows focal, mild interstitial infiltrates of lymphocytes and histiocytes. Spaces among collagen bundles are widened. Collagen bundles in some area are fibrillated (see S5C20aP13a-2). The widened spaces are interpreted as a manifestation of a mucinosis in the setting of a lymphocytic infiltrate of the dermis.

S5C29P22-2: Both the epidermis and the upper portion of the dermis show atrophy. The epidermis is thin with effaced rete ridges. The loosely spaced collagen bundles in the upper portion of the dermis are delicate. A follicle cut in cross-section in the upper portion of the dermis shows a lichenoid pattern at its interface with the follicular sheath. There are minimal perivascular and perifollicular infiltrates in the upper portion of the dermis (Degos disease).

S5C29P22-3: In this field, the dermal collagen bundles are delicate and widely spaced. Mucinous matrix is prominent in the interstitial spaces (Degos disease).

S5C29P22-4: The reticular dermis shows atrophy of the collagenous component. Delicate bundles of collagen are loosely spaced in the area of atrophy. Red arrows point to small vessels with thickened, hyalinized walls. There is an increased amount of mucinous matrix among the atrophic collagen bundles (Degos disease). An atrophic epidermis shows liquefaction degeneration at the dermal-epidermal interface.

S5C29P22-5: Red arrows identify an inflamed venule. Blue arrows identify an occluded vessel, presumably an arteriole. See next photomicrograph.

S5C29P22-6:  A small vein near the top of the field shows mural infiltrates of lymphocytes (venulitis). Near the bottom of the field, a small vessel, presumably an arteriole, shows mucinous hyperplasia of the intima and obliteration of the lumen (Degos disease).
