Dyschromicum Perstans

S4C23P15-1: The epidermis shows slight hyperkeratosis and acanthosis. Rete patterns are irregular; they are mostly effaced. There are perivascular infiltrates of lymphoid cells in the upper portion of the dermis with a few, spotty areas of liquefaction degeneration at the dermal-epidermal interface. The basal layer of the epidermis is variably pigmented; melanophages are present in the upper portion of the dermis ( erythema dyschromicum perstans).

S4C23P15-2: At higher magnification, vacuolar changes are focally prominent at the dermal-epidermal interface. In the center of the field, there is irregular pigmentation of the basal layer of the epidermis. The papillary dermis is widened and fibrotic. There are loose collections of melanophages in the perivascular spaces and in the interstitium in the deeper portion of the papillary dermis ( erythema dyschromicum perstans).

S4C23P15-3: In the areas showing liquefaction degeneration of the basal layer of the epidermis (green arrows), there is epidermal hypomelanosis (erythema dyschromicum perstans).

S4C23P15-4: The patterns qualify as those of a cell-poor lichenoid reaction with pigment incontinence. Red arrows point to migratory histiocytes. Green arrows identify an area showing liquefaction degeneration of the basal layer of the epidermis and epidermal hypomelanosis (erythema dyschromicum perstans).
