S1C24P14-1: Chondrodermatitis nodularis helicis is a lesion with zonal patterns and a rather well defined relationship to a particular anatomic site. In this field, the basic features are well developed. Centrally,
the basal unit of the epidermis is interrupted by a lytic defect; the superficial unit is intact over the defect. There is a zone of fibrinoid necrosis along the floor of the defect (red arrows). In this example,
the defect and the zone of necrosis extend to the plate of cartilage and perichondrium at the deep margin of the specimen (blue arrows); the perichondrium is partially interrupted along the floor of the defect. To
either side of the zone of fibrinoid necrosis, there is a zone of vascularized granulation tissue. Beyond this zone, the dermis shows solar elastosis. The plate of cartilage is outlined by green arrows. Local trauma
probably has a role in the pathogenesis of this disease. Often, the cartilagenous component is not represented on superficial biopsy specimens. Even in the absence of a cartilagenous component, the changes are
characteristic.
