S15C7P1 Superficial Morphea & LSA

S15C7P1-1: The patterns are LS&A-like. There is partial effacement of rete ridges with spotty lichenoid patterns at the dermal-epidermal interface. The papillary dermis is widened  and fibrotic; it is relatively free of lymphoid infiltrates. There are perivascular and interstitial infiltrates of lymphoid cells in the upper portion of the reticular dermis; the infiltrates are confined outside the zone of fibrosis in the upper portion of the dermis. These are all LS&A-like features. The rather fibrous character of the papillary dermis and the upper portion of the reticular dermisare features which deviate somewhat from the edematous hyalin which would be characteristic of this anatomic site in a classic lesion of LS&A (superficial morphea) (see S15C5P1-1-4: sclerodermoid porphyria).

S15C7P1-2: The features are LS&A-like; LS&A would be difficult to rule out. The distinctions between LS&A and superficial morphea are arbitrary.

S15C7P1-3: This is a fairly classic example of LS&A. There is compact hyperkeratosis. Rete patterns are effaced and there are areas of liquefaction degeneration at the dermal-epidermal interface. A superficial portion of the dermis, including the papillary dermis and a portion of the reticular dermis, shows edematous hyalinization, and telangiectasia.

S15C7P1-4: The altered fibrous matrix is edematous and hyalinized. As a common feature, elastica of the reticular dermis is often found in the zone of fibrosis; having identified this feature, an observer can then conclude that the fibrosing process has extended into the upper portion of the reticular dermis ( LS&A).

S15C7P1-5: Patterns at the dermal-epidermal interface are lichenoid and cell-poor (LS&A).

S15C7P1-6: Hyalin is prominent at the dermal-epidermal interface, but basement membrane changes, that resemble those of lupus erythematosus, are not a feature. Vessels are ectatic (LS&A ).
