S15C2P3-Atypical Histiocytoma

S15C2P3-1: This is a lesion from the scalp of a 3 year old boy. The lesion is diffuse in the upper 1/3 of the dermis. Collagen bundles of the reticular dermis are preserved among nests of cells in the lower portion of the field. The lesion presses upon the epidermis. A follicle is preserved within the lesion.

S15C2P3-2:The infiltrate of histiocytes extend to the dermal-epidermal interface. Rete patterns are partially effaced. There are loose infiltrates of lymphocytes, among the histiocytes, on the right side of the field.

S15C2P3-3: In this field, some of the histiocytes have enlarged nuclei; some of these cells have cleaved nuclei (green arrows). The cleaved nuclei resemble the nuclei of histiocytes in the infiltrates of histiocytosis X.

S15C2P3-4: There are scattered multinucleated giant cells (green arrows). Cells are loosely spaced in a fibrous matrix. There are loose infiltrates of lymphoid cells.

S15C2P3-5: Histiocytes are loosely spaced in a fibrous matrix. One giant cell has features of a touton type cell, but each of its nuclei is deeply grooved (atypical histiocytoma or atypical xanthogranuloma).
