S13C14aaP12aa-Porokeratosis

S13C14aaP12aa-1:To the right of the center of the field in the region beneath the green arrows, the epidermis shows hyperkeratosis, effacement of rete ridges, and slight atrophy. To the left beyond the area outlined by red arrows, the epidermis is relatively normal. In the area defined between the red arrows, the epidermis shows parakeratosis with collections of neutrophils in the parakeratotic cap. The epidermis beneath the parakeratotic cap is hyperplastic; it shows vacuolar changes, even at this magnification.The patterns, in the area between the red arrows, is psoriasiform; it is at the margin of the area defined by green arrows. In the area defined by green arrows, the pattern is that of a senescent lichenoid reaction. The relationships, but not all of the patterns, are of type seen in a lesion of porokeratosis. The exception, for this lesion, is found in the area between the red arrows; the changes in this area are psoriasiform in a pattern. A “cornoid lamella,” in a psoriasiform pattern, is not currently recognized as a variation in the setting of disseminated superficial actinic porokeratosis.

S13C14aaP12aa-2: Neutrophils have collected in the zone of parakeratosis; this is a most unusual feature, if this lesion is to be accommodated in the category of disseminated superficial actinic porokeratosis. Perinuclear vacuoles are prominent, but they also are prominent in the epidermis beneath cornoid lamellae in classic examples of porokeratosis.

S13C14aaP12aa-3: In this field, the patterns might be characterized as a psoriasiform cornoid lamella (“disseminated superficial actinic porokeratosis, psoriasiform variant”). There is a “Munro microabscess.” There is nuclear disarray but this is a common feature of the epidermis beneath a coronoid lamella.
