S13C14aaP12aa-1:To the right of the center of the field in the region beneath the green arrows, the epidermis shows hyperkeratosis, effacement of rete ridges, and slight atrophy. To the left beyond the area outlined
by red arrows, the epidermis is relatively normal. In the area defined between the red arrows, the epidermis shows parakeratosis with collections of neutrophils in the parakeratotic cap. The epidermis beneath the
parakeratotic cap is hyperplastic; it shows vacuolar changes, even at this magnification.The patterns, in the area between the red arrows, is psoriasiform; it is at the margin of the area defined by green arrows. In
the area defined by green arrows, the pattern is that of a senescent lichenoid reaction. The relationships, but not all of the patterns, are of type seen in a lesion of porokeratosis. The exception,
for this lesion, is found in the area between the red arrows; the changes in this area are psoriasiform in a pattern. A “cornoid lamella,” in a psoriasiform pattern, is not currently recognized as a variation in the
setting of disseminated superficial actinic porokeratosis.
