S13C17P15-Kaposi Disease (fibrosing)

S13C17P15-1: This lesion is a classic example of Kaposi’s sarcoma, represented mostly in patterns of the fibrotic variant, but with small, lobular components (“promontories”) that are unusually prominent. In the lobular components (blue arrows), the patterns overlap with the patterns of a solid variant of Kaposi’s sarcoma. The patterns are bi-phasic; lobular and interstitial components are represented. A well-developed, interstitial component is outlined by green arrows. The lobular components are composed of immature vessels and aggregates of spindle cells.

S13C17P15-2: In the lobules (blue arrows), vessels, that are lined by spindle cells, are loosely spaced in a fibrous matrix. In the interstitial component (green arrows), angulated spaces among altered collagen bundles are partially lined along one surface by spindle cells.

S13C17P15-3: Blue arrows identify the lobular components (i.e., promontories). Green arrows outline an area in which interstitial changes are prominent.

S13C17P15-4: In the interstitial component, angulated, anastomosing spaces among collagen bundles are lined by flattened spindle cells in a spotty fashion. The patterns have a lymphangio-endotheliomatous quality.

S13C17P15-5: Lobular and interstitial components are represented. In the lobule, there are “extravasated” red blood cells among spindle cells.

S13C17P15-6: In this “promontory,” spindle cells are focally clustered. Slit-like spaces among the spindle cells contain red blood cells. In this area, the patterns overlap, in part, with those of the solid variant of Kaposi’s sarcoma.

S13C17P15-7: Slit-like spaces, among closely spaced spindle cells, contain red blood cells. There are scattered hemosiderin deposits (Kaposi’s sarcoma).
