S12C12cP2c-1: The nuclei of some of the activated histiocytes are deeply notched.

S12C12cP2c-2: In the subcutaneous fat, small focal areas show a lipogranulomatous quality with a high component of histiocytic giant cells. The large, empty space to the right contributes a xanthogranulomatous quality to the pattern. An area of suppuration is represented in the right lower corner; the process is a suppurative granuloma. Special stains would be indicated.

S12C12cP2c-3: Acid fast organisms vary in size. Some are long and irregular in outline. One of the organisms is beaded; the beads are not a prominent feature of the photomicrograph at this magnification. The variability in size of organisms is a feature of some examples of atypical acid fast infections. Beading of organisms may be a feature of degeneration.

S12C12cP2c-4: Acid fast organisms vary in size (mostly length). They often are much longer than the organisms encountered in either tuberculosis or leprosy.

S12C12cP2c-5: Organisms are clustered near this area of suppuration.

S12C12cP2c-6: On a gram stain, organisms to not appear to be as numerous; many are gram negative (red arrows). The gram negative organisms may be degenerating forms.

S12C12cP2c-7: With a silver-methenamine stain, some of the organisms are positive.
