S12C11P1-Sarcoidosis & Lupus Vulgaris

S12C11P1-1: Tubercle-like aggregates of histiocytes are collected in the adventitia of dermal vessels. Lymphoid infiltrates are inconspicuous; the patterns are those of “naked” tubercles (epithelioid granulomas) ( sarcoidosis).

S12C11P1-2: Aggregates of epithelioid histiocytes are loosely spaced in the dermis and are associated with infiltrates of lymphoid cells. The granulomas are associated with multinucleated giant cells ( lupus vulgaris). Initially, this patient had been diagnosed as having sarcoidosis  and was followed for several years. Following treatment with adrenocortical steroids, acid fast organisms were found in a skin biopsy and the diagnosis was changed to lupus vulgaris. The patient had received x-irradiation and subsequently developed a squamous cell carcinoma in the site of the lupus vulgaris and irradiation.

S12C11P1-3: In this field of a lesion of lupus vulgaris, the infiltrates are somewhat disorganized. Tubercle-like aggregates are not a prominent feature; fibrosis is a feature.

S12C11P1-4: Two acid-fast organisms are present near the center of the field (lupus vulgaris).
