S11C14P9-1: Perineal granuloma inguinale.

S11C14P9-2: The epidermis is hyperplastic in psoriasiform patterns (there is hyperplasia of the superficial and basal units of the epidermis with elongation of the rete ridges). The upper portion of the dermis is edematous; infiltrates of inflammatory cells are diffuse.

S11C14P9-3: The infiltrates near the dermal-epidermal interface are composed of histiocytes  and scattered clusters of neutrophils. (blue arrows). The histiocytes are loosely spaced; they are not tightly arranged in epithelioid patterns.

S11C14P9-4: This pattern probably represents a dermal papilla cut in cross-section. In the papilla, there are scatttered neutrophils. Histiocytes are of two types: 1.) migratory forms with scanty cytoplasm and elongated, wavy nuclei; and 2.) activated histiocytes with pale acidophilic cytoplasm  and eccentric nuclei.

S11C14P9-5: Histiocytes and neutrophils (blue arrows) are admixed. The histiocytes are activated, but are not arranged in epithelioid patterns.

S11C14P9-6a, b, & c: sections of lesions showing Donovan bodies at various magnifications (Warthin-Starry stain; a silver impregnation technique). It would be difficult to characterize any of the organisms as having the appearance of a “closed safety-pin.” To the left in figure P9-6c (third, or bottom photo ), organisms are marginated in a cytoplasmic vacuole. Centrally and to the right, the organisms are extracellular.
